I understand the Fisher Group requires physical screenings, in order to determine my ability to perform the essential functions of the position for which I am applying. The examination includes drug use analysis. I authorize investigation of all statements contained in this application. I understand that misrepresentation or omission of facts called for on my employment application materials is cause for dismissal. I also understand I may be assigned to positions other than that for which I initially made application and that my work hours may be changed, I agree that if employed by the Fisher Group, in consideration for such employment, I shall become familiar with and comply with policies, procedures, and safety practices of the Fisher Group and may be terminated by the Fisher Group at any time for any reason or no reason. I understand that I have not been promised or assured of employment for any specific period of time. I authorize persons, schools, current employer (if applicable) and previous employers or organizations named in this application (accompanying resume, if any) to provide the Fisher Group with any relevant information which may be required to arrive at an employment decision. I authorize the reference sources, including previous employers, listed in this application to provide the Fisher Group with ALL information pertinent to this application and I release all parties from liability for any damages and any claims that may result from the release of any information as a part of the employment verification process.
